Registering with the surgery
To register with the surgery:
- Register online – use the NHS register with a GP surgery: St Georges Medical Centre service
Please note, we may ask you to visit the practice to provide additional documentation needed to complete your registration, as well as the new patient health questionnaire (PDF).
- For children – Due to safeguarding, we will request you to come into the practice for children
- For children under 5 – Please bring in red book or record of immunisations to practice
You can also:
- visit reception from 10am to 6pm and collect a paper registration form
- download a paper new patient registration form and our new patient health questionnaire (PDF) to print and fill out
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Temporary residents
If you have visitors who need urgent treatment we may be able to see them as temporary residents. Please ask at reception.
If they are visiting from abroad, certain countries have reciprocal health agreements but visitors from other countries may have to pay for our services.
